Individual coverage

 

This is the most basic health insurance plan offered by Star Health insurance. The benefits offered under this plan are:

This policy offers coverage in the range of Rs.1.5 Lakh and Rs.15 Lakh for individuals between age 5 months and 65 years.

Star Unique Health

This is only plan, which covers HIV patients under some terms and conditions given below:

This plan offers an individual coverage in range of Rs.1 lakh to Rs.3 lakh for individuals between age 18 years and 65 years.

Policy period is two years, which allows premium payment in installments.

First installment -policy commencement

Second installment – beginning of second policy year.

In this plan, the premium value differs with different locations and there is a copay of 30% (amount to be paid by the individuals themselves for each claim).

The policy covers HIV patients with CD4 count above 350 at the time of buying the policy.

Star Super Surplus

Individual Plan

 

This is a top-up plan, which covers medical expenses after the expenses exceed the deductible amount. The plan offers basic medical coverage. Some details of coverage offered are:

This policy has a deductible feature (fixed amount to be paid before claiming against the insurance) and is available in three options.

Option I- Insurance of Rs.7 lakh and a deductible of Rs.3 lakh

Option II- Insurance of Rs.10 lakh and a deductible of Rs.3 lakh

Option III- Insurance of Rs.10 lakh and a deductible of Rs.5 lakh

Individual between ages 5 months to 65 years can buy this policy.

Family Floater Plan

 

This is a family floater, which covers health costs of the entire family. This plan requires a minimum deductible amount to be paid by the family. The plan is available in two options.

Option I- Insurance of Rs.10 lakh and deductible Rs.3 lakh

Option II- Insurance of Rs.10 lakh with the deductible of Rs.5 lakh

Star Criticare Plus

This is a benefit plan, which offers lump-sum benefit after the insured is diagnosed with any of the listed critical conditions.

This plan offers an individual coverage in the range of Rs.2 lakh and Rs.10 lakh for individuals between age 18 years and 65 years.

A lump sum amount equal to the sum insured would be paid to the policyholder in the first instance of critical illness claim.

Diseases like chronic kidney disease; major organ transplant etc. are categorized as critical illnesses.

Diabetes safe

Individual Coverage

 

This is a unique plan available only for diabetic individuals.

This plan is for individuals who have diabetes. It offers coverage in the range of Rs.3 lakh and Rs.10 lakh

The plan does not have a waiting period for diabetes-related health issues.

There are two variants of this plan.

Plan A- Has no waiting period. There is a pre-medical examination requirement for this plan. Diseases other than diabetes-related illnesses are covered like all other regular health plans in the market (with applicable waiting periods as defined in the policy)

Plan B- Has a waiting period of 30 days but no pre-medical examination is required. Diseases other than diabetes-related illnesses are covered like all other regular health plans in the market (with applicable waiting periods as defined in the policy).

Family Floater

This plan is available for coverage of diabetic proposer and diabetic spouse.

This is family floater plan, which covers health costs of the entire family in range of Rs.3 lakh and Rs.10 lakh.

It is specifically for individuals who have diabetes and their spouse. No other member of the family will be covered in this plan.

The plan covers regular hospitalization along with any complications related to diabetes without any waiting period.

There are two variants of this plan.

Plan A- Has no waiting period. There is a pre-medical examination requirement for this plan.

Plan B- Has a 30 day waiting period. However, there is no pre-medical examination requirement.

Star Cardiac Care

This is plan specifically available for individuals with complications related to heart. Details of this plan are:

It is one designed especially for individuals suffering from heart disease.

It offers coverage in range of Rs.3 lakh and Rs.4 lakh for individuals between age of 10 years and 65 years who have undergone PTCA (Stenting) or CABG (By-pass) within a period of 4 years before buying this policy.

In this plan, there is a waiting period of just 90 days for cardiac related illnesses. For all other illnesses, regular waiting periods, (as defined in the policy) apply.

This plan is available in two variants

Silver– Covers only complications, which require surgery or intervention.

Gold– Covers all cardiac related illnesses, which may or may not require surgery or intervention.

Claims process at Star Health and Allied Insurance

There are two ways to make claim under a Health Insurance Policy: on cashless basis and on reimbursement basis. In simple terms, for making a claim on cashless basis, the treatment must be done only at a network hospital of the insurance company servicing your policy. For availing the treatment, you would have to first seek an authorization as per the procedures laid down and in the prescribed form. In case of claims on reimbursement basis, the insurance company has to be informed as per their prescribed procedures. The policyholder has to ensure that documents such as claim form, discharge summary, prescriptions and bills to be submitted for the claim on reimbursement are obtained after hospitalization.

Cashless Service: seeking Pre-Authorization

 

Complete the pre-authorization form available at the hospital’s insurance/TPA Desk.

Send the completed form via email or fax.

Approved letter will be sent by the claim management team. Remember that the claim management team could send an inquiry to the hospital or you before authorization.

In case the authorization is rejected, initiate the treatment and file for reimbursement claim.

Reimbursement Claim: Claim Submission

 

Complete the necessary form and submit along with required documents, as per the policy terms and conditions.

Necessary documents include discharge summary, prescriptions, bills etc.

Approved letter will be sent by the claim management team. Remember that you will be answerable to any queries raised by the claim management team before the letter is issued.

In case the claim is rejected, the reasons will be communicated by the team.

In case of any emergencies, you need to inform Star Health within 24 hours of the admission. In case of planned emergencies, the notification has to reach the company 48 hours prior to the hospitalization.
